Client Intake Form

If you are a first-time client, please print this form, complete it, and bring it along to our first session. You can also email the completed form to us if that is easier for you.

Intended Outcomes Form

This form will help you clarify your goals for your overall health and vitality, including our time together. You can either bring this form completed to our consultation or you can email it to us.

3 Day Dietary Record

Keep track of everything but water for 3 consecutive days to help me understand your normal intake.

All information i collect is for use in creating your fitness plan only.
